Direct Payment Support Service

The Direct Payment Support Service will enable Individuals eligible for social care funding to exercise choice and control over their care and support arrangements. The Council is committed to ensuring that the option of a Direct Payment is explored and understood by everyone who is eligible to receive one. The Service will be available to an Individual who is eligible for social care services and chooses to take part or all of their Personal Budget as a Direct Payment to employ their own Personal Assistants or pay for other services of their choice. The Provider shall deliver the service to; Children and Adults eligible for Direct Payments, which may include, but not be limited to: -Older people. -People with physical disabilities, learning disabilities, autism, or mental health needs. -People with fluctuating or complex needs. -Individuals transitioning from Children's to adult services. -Carers (including parent carers) managing Direct Payments on behalf of Individuals.

Provision of Personal Care and Support to Tenants in Extra Care Housing

Darlington Borough Council

Requirement of Personal Care and Domestic Support for tenants living in Extra Care Housing. The service is to provide regulated personal care and domestic support to individuals with assessed care and/or support needs who are living within Extra Care Housing settings across the borough of Darlington. The Council's vision is to reduce reliance on residential care by enabling individuals to access high-quality care and support within their own homes. The service will support people to maximise their independence and wellbeing, ensuring they receive the right care at the right time, and are empowered to contribute to and feel valued within their communities. Scope of the Service The service will be available to individuals including: • Person aged 55 and over, however younger individuals may reside where their Council-led Care Act assessment has identified a need that can be best met within the Extra Care environment. • Person with dementia • Person with a physical disability • Person with a sensory impairment • Person with a long-term condition • Person with a learning disability • Person with a mental health condition • Person with bariatric needs, accessibility will be considered with respect to allocation of properties. • Person with palliative care needs • Person with behaviours that may challenge • Carer where their Council-led Carers assessment has identified a need to help them manage daily life while continuing to provide care. Geographical Scope and Current Provision • There are currently 4 Extra Care schemes operating within Darlington, providing a total of 154 units across the borough • The accommodation comprises a mix of Council-owned and social rented tenures, supported by a commissioned regulated care provider • Darlington Borough Council housing operates 3 schemes, and works in partnership with Anchor Housing on additional scheme.
