Stoke-on-Trent City Council is seeking to appoint one Provider for Lot 1 and one Provider for Lot 2 to deliver two specialist short-term home-based support services for adults aged 18 and over. The services are designed to support people to remain safely within their own homes during periods of increased need, crisis, recovery, assessment or transition, whilst avoiding unnecessary hospital admissions and long-term residential care placements. Stoke-on-Trent City Council is undertaking a procurement exercise for a Community Support Framework, for children living within Stoke-on-Trent and for children who may be supported in other areas of the country. The Council wishes to source multiple providers to deliver the services. The services will stabilise children and families who have support needs that require statutory intervention.
