Description
Devon County Council is commissioning services under the My Future, My Way model. This support will operate across a range of settings, including 'Homes with Support' - where staff may be based on-site in both regulated and unregulated services - and Community Based Support. Community Based Support will extend beyond the home as a young person moves on, offering places they can visit for help, as well as opportunities to develop independence in their own accommodation (such as through Housing First etc). Suppliers will need to work in active partnership with young people, families, and professionals to co-create solutions that promote safety, stability, and progression to independence. The specification reflects a value-based approach within a framework, emphasising relational practice, trauma-informed support, and inclusive service delivery that supports young people to thrive.
